A referral can be made by either a parent\/guardian or a school staff member.<\/p>\n<p>Our district uses a <a href=\"\/index.php\/multi-tiered-system-of-supports\/\"><strong>Multi-Tiered System of Support (MTSS)<\/strong><\/a> to help students.<\/p>\n<p>In most cases, students are first referred to the <a href=\"https:\/\/www.nj.gov\/education\/njtss\/resources\/irs\/\" target=\"_blank\" rel=\"noopener\"><strong>Intervention and Referral Services Team (I&amp;RS)<\/strong> <span class=\"ext\" aria-label=\"opens in a new tab\">(external)<\/span><\/a>. This team works with teachers and families to provide extra support. If, after these interventions, a student continues to struggle, the team may decide to:<\/p>\n<ul>\n<li>\n<p>adjust the current support plan,<\/p>\n<\/li>\n<li>\n<p>review the student for 504 eligibility, or<\/p>\n<\/li>\n<li>\n<p>recommend a referral to the <strong>Child Study Team<\/strong> for a full evaluation.<\/p>\n<\/li>\n<\/ul>\n<p>If you would like to request a meeting with the Child Study Team to discuss a possible evaluation for your child, you may:<\/p>\n<ul>\n<li>\n<p>fill out the <a href=\"https:\/\/docs.google.com\/document\/d\/1jNXoxYILt2lXX_NKmLJ1_J1BnyLX-KlV\/edit\" target=\"_blank\" rel=\"noopener\"><strong>Referral Form<\/strong> <span class=\"ext\" aria-label=\"opens in a new tab\">(external)<\/span><\/a>, or<\/p>\n<\/li>\n<li>\n<p>send an email to <strong>Crystal Gonzalez, Child Study Team Secretary<\/strong>, explaining your reason for the request.<\/p>\n<\/li>\n<\/ul>\n<p>Please remember to <strong>sign the bottom of your form or letter<\/strong>. Referrals must include an original signature.
